Landlord Forms
Submitting forms:
- Almost all forms may be submitted by email to rentprogram@alamedaca.gov.
- Many common forms can be submitted online by logging in to the Alameda Rent Registry.
- You may contact staff to request a link to complete the form via DocuSign.
- Forms may be submitted by U.S. mail.
- A drop box in the lobby of City Hall West, 950 W. Mall Square, is accessible during business hours Monday through Thursday. [map]
- A 24-hour drop box is located on Oak Street between City Hall and the Alameda Police Department. [map]
RENT INCREASE PETITION: A landlord may use these forms to request a hearing for an upward rent adjustment above the Annual General Adjustment or to contest the Maximum Allowable Rent.
TERMINATION NOTICE FOR OWNER MOVE-IN: A landlord must file this form after serving a tenant with a termination of tenancy notice for owner move-in.
TERMINATION NOTICE FOR WITHDRAWAL FROM THE RENTAL MARKET: A landlord may use these forms to proceed with a termination of tenancy to withdraw a property permanently from the rental market.
BANKING ADDENDUM: A landlord must use this form if serving a notice of rent increase that includes a banked Annual General Adjustment (AGA).
PROOF OF SERVICE: A landlord may use this form to document that a tenant has been served any documents required by the Rent Ordinance.
BUYOUT OFFER: A landlord must use this form to serve a tenant with a buyout offer.
PERMANENT RELOCATION PAYMENT NOTICE: A landlord may include this document with a notice to permanently terminate a tenancy for owner move-in, demolition, withdrawal from the rental market, Capital Improvement Plan, or compliance with a governmental order.
TEMPORARY RELOCATION: A landlord must use this notice in the following situations: 1) When the landlord takes action to terminate a tenancy temporarily; 2) When the tenant has temporarily vacated the rental unit (A) in compliance with a governmental agency's order to vacate; (B) due to health or safety conditions, as defined; (C) as part of an approved Capital Improvement Plan.
DISCLOSURE TO NEW TENANTS: A landlord may use this form to provide notice and materials to a tenant regarding the City of Alameda's Rent Program.
RELOCATION RENT INCREASE NOTICE: A landlord must use this form when serving a tenant with a "Relocation Rent Increase".
NOTICE OF ANNUAL GENERAL ADJUSTMENT: A landlord may, but is not required to, use this template to notify tenants of an AGA rent increase; a banked rent increase; and other increases, adjustments or pass throughs permitted by the Rent Ordinance.
DECLARATION OF COMPLIANCE FOLLOWING TERMINATION FOR OWNER MOVE-IN: A landlord must use this form to verify that the landlord is in compliance with the unit restrictions following a termination of tenancy for owner move-in.
DECLARATION OF COMPLIANCE FOLLOWING TERMINATION FOR WITHDRAWAL FROM THE RENTAL MARKET: A landlord must use this form to verify that the landlord is in compliance with the unit restrictions following a termination of tenancy for withdrawal from the rental market.
CAPITAL IMPROVEMENT PLAN: Landlords may use this form to request approval of a Pass Through for eligible property improvement costs, and(or) request conditional approval to temporarily relocate a tenant(s) because of the work associated with eligible improvements.
CHANGE IN OWNERSHIP: Landlords may use this form to notify the Rent Program of a change in ownership and associated information that must be reported within 30 days of the close of escrow for all rental units subject to rent control. (Alternately, sellers who previously set up a Rent Registry account and associated it with the APN may submit the information online.)
REGISTRATION - FULLY REGULATED UNITS: Landlords may use these forms to submit the necessary information to register rental units that are subject to rent control, i.e. at a multi-unit property built prior to February 1995. (Alternately, landlords may submit registration information online via the Rent Registry. See the User Guide for details.)
NEW FORMS including RP-221(B) for submitting annual registration information:
REGISTRATION - PARTIALLY REGULATED UNITS: Landlords may use these forms to submit the necessary information to register a condominium or single-family home that is rented to a tenant or a rent-subsidized tenant. (Alternately, landlords may submit registration information online via the Rent Registry. See the User Guide for details.)
NEW FORMS including RP-223(A) for submitting new tenancy registration for partially regulated units.
REASONABLE ACCOMMODATION: This form may be used by clients or applicants to request a reasonable accommodation so that an individual with a disability may have equal opportunity to use and enjoy participation in any of the programs conducted by the Rent Program.
FEE PAYMENT: Before the annual program fee is due, landlords will receive an invoice in the mail with instructions on how to pay by check, or they are able to pay by credit card using the online Rent Registry.
